The following forms are used for Enrolment for Instrumental and Speech Lessons at the Southport School.

Brochure with Enrolment Form - To enrol for small group/private instrumental music and speech lessons
Change of Enrolment Form - Used to request changes in instrumental or speech lessons (eg: small group to private) or to cancel enrolment. ** Please see enrolment conditions.
